Product Release Notes: Fama Consent Forms

We are excited to announce the launch of Fama Consent Forms – a premium service that streamlines the candidate consent process for Fama checks! This enhancement ensures compliance and automates it by collecting candidate consent collection before a report is run.

How It Works

  • Feature Enablement: For Fama Plus and Essential customers, Consent Forms is a Company and/or Sub-company setting. Once enabled by Fama, all reports run by the Company and/or Subcompany will require the signed consent forms prior to running the Fama check. For Fama 360 customers, simply reach out to your Account Manager to enable this feature.
  • Adding New Reports: For Fama Plus and Essential customers, there is no change. To request new reports, simply continue to use the Add Report form in the app.fama.io web application or run your API as usual. For 360 Customers, continue to request reports via the 360 web app and they will automatically include candidate consent once enabled.
  • Once a new report is submitted: An email with the disclosure and consent forms will be sent to the email inputted in the “Email Address” box in the Add Report Form.
  • Candidate Consent: Candidates will receive an email from noreply@mail.hellosign.com asking them to read the Fama disclosure forms and sign the consent forms. All candidates will receive the same email below. Fama will notify customers when there are any changes to the email. Note the customization outlined in red. The green outline is where candidates will click to complete the disclosure forms. The signature process is powered by a compliant 3rd party, Dropbox Sign.
  • Consent Form Status: Fama Plus and Essential customers can check the status of a candidate’s consent forms within the app.fama.io web application on the new Consent Form page. You will see status updates, including: Sent âž” Viewed âž” Signed âž” Ready.

Candidate Reminders: Candidates will receive a reminder email after three days and 7 days until the consent form is completed. Customers can manually send consent forms by clicking the “Resend” button next to the candidate’s name on the Consent Form page.

  • View and Download Consent Forms: Plus and Essential customers can view and download signed candidate consent forms on the Consent Form page by clicking the link next to the candidate’s name under the “View Form” column. 


360 customers using the Fama Consent Form feature will be able to download their consent from in the 360 web application, next to the Download Report button. 

Troubleshooting Notes 

  • Candidate Consent Forms is a premium feature and will be available to Customers for an additional fee.
  • Customers can see whether the Consent Form feature is enabled by looking for the green “On” sign on the Company and/or Subcompany Edit Page.

  • Once subscribed, Consent Forms will be sent to all candidates run by the Company and/or Subcompany. This is an all or nothing feature. 
  • Customers using the Fama Consent Form feature must use Fama’s standard Consent Forms. Consent Forms cannot be customized at this time. 
  • Customers running Fama reports through an API can use this feature.
  • Turnaround Time (TAT) will not begin until Consent Forms have been signed. 
  • If the consent form workflow breaks, please cancel the report request and create a new report using the Add Report form. Additional billing may apply.  
  • If you need to update a candidate’s email address, please cancel the report request and create a new report using the Add Report form. Additional billing may apply. 
  • Consent Forms are U.S. specific forms. 

Who Does This Impact?

All Fama customers are eligible for the Consent Form feature. Fama Essential and Plus customers will be able to check for status updates, manually resend reminders to candidates, as well as view and download consent forms from within the app.fama.io web application. 

Fama 360 customers can reach out to operations@fama.io to check on the status of consent forms. Signed Consent Forms will be available on the 360 web application.
